Improving Your Gait and Reducing Injuries When it comes to running and physical activity, our bodies often reveal unique characteristics and imbalances in movement. One way to identify and address these issues is through a detailed biomechanical analysis of your gait. This specialised analysis, conducted by podiatrists, can uncover abnormalities and imbalances that may be affecting your running form and increasing your risk of injury. During a biomechanical analysis, podiatrists will carefully observe and assess your gait, looking for any signs of irregular movement or imbalances. By examining factors such as foot strike, stride length, and joint alignment, they can gain valuable insights into how your body moves while running or walking. Gait Analysis The PodoSmart system is a wearable sensor technology that accurately measures multiple gait parameters in the lower limb relating to real-life conditions. It comprises a pair of intelligent insoles, coupled to an easy-to-use web interface, which allows clinicians to view the data recorded by the PodoSmart system. The intelligent insoles feature sensors designed to detect tiny foot movements. These data points can be used to monitor foot injuries and detect abnormalities. Each Podosmart insole has an onboard inertial unit recording the movement and orientation of the foot in space. This data is processed by artificial intelligence algorithms to recognise walking steps, running strides and other activities and then calculate the space-time, kinematic and biomarker parameters displayed in the interface. The insoles are placed in the patient’s shoes, and then they are asked to walk, or run, in the shoes. This can be performed either indoors or outdoors. After the patient is done walking or running, the data is directly sent from the connected insoles to the web interface on the computer with the help of Bluetooth signals. The device provides a detailed review of how a person walks by providing data on the following points: Cadence Speed (walking, propulsion and average) Digital Gaitline Dynamic pronation/supination Time (stance, stance phases, swing, double support, stride) Stride length and asymmetry What’s Next? Once the analysis is complete, we can then provide recommendations to improve your gait and reduce the risk of injuries. This may involve making adjustments to your running form, suggesting specific shoe types that can better support your feet, or prescribing custom orthotics to address underlying biomechanical issues. Custom Orthotics Using 3D modelling software, we are able to design and produce our own customised orthotics here at Gait Way Podiatry. It starts with a dynamic scan on a baropodometric platform to detect the patient’s foot loads and posture during standing and walking. The foot is then scanned with the 3D Podoscan, an electronic podoscope for acquiring the digital footprint. The orthotic is constructed using the information gathered from the previous steps. The podiatrist’s preferences for control and accommodating features are then added. Following this, the orthotic is milled directly on our premises, and any additional padding, wedging and top-covers are added. It can be produced within an hour, however, we will allow for a day to make it the best quality possible. Runners, athletes and those competing in various levels of sport can now have the thinnest and most structured orthotics made to reduce the risk of injury or re-injury. By using this 3D modelling system, it means that the orthotics can be better fitted to your athletic footwear. This allows us to deliver a high-quality product in half the time. The Right Shoes As well as the terrain underfoot, you must select the right pair of shoes to go the distance you require. If you’re training for a marathon or event, you’ll require a different running shoe than running a 5k in the park. Long distances require a more cushioned shoe, while shorter, everyday runs need a more flexible training shoe. Footwear is an integral tool to use in maintaining our foot health and running comfort. - PROLOTHERAPY
What is Prolotherapy? Prolotherapy is the shortened version of “proliferative therapy”. It is the injection of healing substances into areas of tendons and ligaments near their attachments to bone. The most common solution used to trigger the healing process is concentrated dextrose (above 10%, typically 15-25%) which is sugar (glucose) in sterile water. What conditions will it help? Prolotherapy is a regenerative injection therapy technique used for many different types of musculoskeletal pain including Osteoarthritis, tendonitis, partially torn tendons, ligaments & cartilage, heel spurs & plantar fasciitis, neuromas, Osgood Schlatter’s Disease, Shin Splints, medial knee pain, foot & ankle pain/injuries, knee injuries, deep aching pain, sports injuries. How does prolotherapy work? A concentrated dextrose solution is injected into the tendon/ligament attachment and results in a short increase in inflammation in that area. This in turn results in inflammatory cells triggering a “healing cascade” of substances the body makes. Fibrin, collagen and other renewing substances form; the result is healthy new tissue. This in turn “tightens” up the ligament or tendon so that it can do its job of supporting the joint properly, resulting in better function and less or no pain. How quickly does it work? You may have some pain and stiffness over the next 48 hours. After that, the pain begins to lessen. However, healing continues long after the injection, which is why the appointments are spread out about a month apart. How often will I need to come? Everyone is different and comes with a different condition and a different degree of pain. Some people occasionally only need one appointment. Others might need ongoing treatment as they gradually progress. The range is around 3-8 treatments. How many injections will I get at any one time? This varies from person to person and will depend on the extent of the pain. But typically, there are several injections at one visit, as all the painful areas of the affected joint(s) are treated. Will it hurt? Local anaesthetic helps to decrease the sensation of the prolotherapy injection. There is also some local anaesthetic included in the dextrose solution (as in “freezing” at the dentist) , which quickly reduces local pain. Sometimes a pain medication or cream will be needed over the next 48 hours. We will do everything we can to make the injections as comfortable as possible. Are there any risks associated with prolotherapy? There are very few side effects or complications. However, some patients may experience a little light-headedness (not serious) or nausea (rare).There may be some bruising (not serious) or a little bleeding/oozing which will stop by applying some pressure.Some treatment stiffness may occur from the process of having an injection. Let us know if any of the injection sites become increasingly red or painful. Temporary tingling or numbness from the medication iscommon, but it is very rare for any numbness to persist. There may be a temporary increasein pain. If you develop sudden shortness of breath, significant chest pain or major trouble breathing, you should immediately go to the nearest emergency room. What is the cost? The cost depends on the time required for the examination and injections, so please ask us about this. Typical prolotherapy treatments cost between $95 and $400. (excluding the consultation) How will I feel after treatment? Initially, you should notice a decrease in pain and an increase in your range of movement. Then as the local anaesthetic wears off, you could well be stiff and a bit sore for a few days. Remember, we are trying to stimulate the inflammatory (and healing) cascade of substances in your body, so this is in fact a good thing. Don’t plan on being too active for a day or two, to allow this process to have its maximum effect. Walking is fine. If you need some pain medication during this stage, we will advise you on what to use. Do not use heat or ice after treatment, we want to stimulate inflammation in the early stages. VERY IMPORTANT INFORMATION! Prolotherapy (as mentioned in “how does prolotherapy work” above) causes a temporary inflammation in the tissues which is crucial in triggering the healing process. It is therefore very important not to take anti-inflammatory medication for a week prior to and after the injections. Anti-inflammatory medication prevents the prolotherapy from working. These medications include the commonly available medications Advil, Ibuprofen, Aleve, Naprosyn, Celebrex, Voltaren and other similar medications. Our podiatrists are happy to provide these eligible patients with DVA Podiatry. Foot injuries are quite common due to rigorous activities like running, jumping, and carrying tactical backpacks or other heavy loads in heavy military boots. Understanding these injuries, their prevention, and treatment is crucial for maintaining the health and readiness of our current service and former members. Over time, combat boots have gone through numerous iterations. The reason for this is simple: with each new battle, it became clear that your footwear can either make or break you. Our podiatrists handle everything from skin and nail care, footwear recommendations and fitting, to mobility assessments, diabetic care, fungal nail treatment, and custom orthotics. We have the skills and experience to help with many lower limb conditions. Although we do offer in-home services for some DVA patients, it is highly advised that in-clinic visits are more effective with the right tools at hand to help us provide the best assessments and treatments. How do I get a DVA Podiatry referral? DVA Podiatry referrals are available from your GP when they complete a D904 form. The D904 referral entitles you to 12 months of Podiatry care, or 12 Podiatry appointments, whichever comes first. Therefore, if you use 12 appointments before the end of the 12 months, you will need to get a new D904 referral from your GP to continue treatment. There is no limit on the number of referrals you can receive from your GP, and we’ll always be sure to let you know if you need a new one.
